OAWA One-Day Membership Form
PLEASE NOTE: THIS FORM IS TO BE USED ONY FOR ONE-DAY OAWA MEMBERSHIPS FOR IDENTIFIED SANCTIONED COMPETITIONS. SIGNING THIS FORM MAKES YOU ELIGIBLE FOR PARTICIPATION AS AN OAWA MEMBER FOR THE EVENT IN QUESTION ONLY. ONE-DAY MEMBERS RECEIVE LIMITED MEMBERSHIP BENEFITS. COVERAGE INCLUDES LIABILITY INSURANCE (for the event only),
ONE-DAY MEMBERS ARE NOT ELIGIBLE FOR MEDICAL or OTHER INSURANCE COVERAGE.
ONE DAY MEMBERSHIPS COST $25.00 AND CAN NOT BE APPLIED TO A FULL OAWA MEMBERSHIP.
PLEASE SEND A COPY OF YOUR PROOF OF AGE TO registrations@oawa.ca or provide a hard copy to the organizer of the event you are entering.
Send your payment for your $25.00 one-day membership by e-transfer to admin@oawa.ca. Once paid, the fee can not be refunded if you change your mind about entering the event in question. Please provide the date of the event you are registering for on the form, and include the name of the person registering on the e-transfer. The $25.00 one-day fee can not be used towards the purchase of a full time membership.
